Bolton were handed the early lead thanks to a defensive error from Carlos Cuellar, who passed the ball straight to Chung-Yong Lee and he gratefully drove the ball into the back of the net.
Sunderland had their fair share of possession, forcing a series of corners, but failed to really test Andy Lonergan before the break.
The Championship outfit doubled their lead just after the restart, Marvin Sordell looping a header over Simon Mignolet and an upset seemed on the cards.
However, substitute Connor Wickham pulled one back for the Black Cats after an hour, shooting through a crowded penalty area.
Martin O'Neill's men then began to turn the screw, levelling the score at 2 each with 15 minutes to go, Craig Gardner giving Lonergan no chance with a thunderbolt from outside the area.
Bolton were under the cosh for the remainder of the match, but managed to hang on to earn a trip to the Stadium of the Light for the replay.
