The Championship side took the lead at Villa Park and a shock looked on the cards in the Midlands as Michael Chopra's squared ball caused a goalmouth melee, with the ball eventually going in off Ciaran Clark.
The visitors could have doubled their lead minutes later, but Chopra was denied by a fine save from Shay Given.
Villa were looking in danger of a meek cup exit but Darren Bent scored his first goal since September just after the break to start the fight-back, with a sweet right-foot finish from Barry Bannan's through-ball.
The Premier League outfit then snuck the win in the 83rd minute, Gabriel Agbonlahor whipping in a cross from the right and substitute Andreas Weimann netted with a glancing header.
