McCarthy injured the hamstring when he came on as a substitute in the 1-0 loss to Brazil on September 7.
He was sidelined from club duty for Orlando Pirates for several months, but returned for the last couple of matches before the Christmas break.
However, it has now emerged that the 35-year-old is struggling to completely overcome the injury.
At this stage the extent of the concern is not completely clear, but coach Gordon Igesund is a worried man; not only over McCarthy, but also over defender Erick Mathoho, who has a knee problem.
"It is a blow when any player called up for the national team pulls out due to injury. It would have been a great situation to have both of them here," said Igesund.
